TRUST REVEALS TERMS OF REFERENCE FOR RCS REVIEW





In October 2015 the University Hospitals of Morecambe Bay NHS Foundation Trust invited a service review of the trust’s urology surgical service by the Royal College of Surgeons (RCS).
This invitation was to review and analyse the quality and safety of the service after the avoidable death of Peter Read from Morecambe at the Royal Lancaster Infirmary.
In 2015 Mr Read died after developing urosepsis caused by ‘missed opportunities to change his ureteric stent’, a coroner later ruled.
Mr Read was treated by Ashutosh Jain, the same consultant who, in 2011, was involved in the care of Irene Erhart from Walney along with Kavinder Madhra.
A coroner said the failings of Mr Jain and fellow urological consultant Muhammad Naseem in not replacing a stent contributed to Mr Read’s death.
The RCS review took place in January 2016 which made several recommendations for the trust.
In July of this year a number of concerns about the review were raised by whistle-blower Peter Duffy who was constructively dismissed from UHMBT in 2016 after he reported incidents involving Mr Madhra, Mr Naseem and Mr Jain.
Mr Duffy, along with Mr Read’s family, claim the review did not sufficiently investigate the events leading up to his death.

Urologist's support for ex colleague who is at risk of being struck off As Kavinder Madhra's Medical Practitioners' Tribunal Service hearing continues, following concerns being raised by the General Medical Council, one of his former colleagues has submitted a character reference.  Last month the panel heard Mr Jain, who is also being investigated, has submitted a character reference in support of his former colleague’s bid to continue working as a doctor.
