TRUST REVEALS TERMS OF REFERENCE FOR RCS REVIEW In October 2015 the University Hospitals of Morecambe Bay NHS Foundation Trust invited a service review of the trust’s urology surgical service by the Royal College of Surgeons (RCS). This invitation was to review and analyse the quality and safety of the service after the avoidable death of Peter Read from Morecambe at the Royal Lancaster Infirmary. In 2015 Mr Read died after developing urosepsis caused by ‘missed opportunities to change his ureteric stent’, a coroner later ruled. Mr Read was treated by Ashutosh Jain, the same consultant who, in 2011, was involved in the care of Irene Erhart from  Walney  along with Kavinder Madhra. A coroner said the failings of Mr Jain and fellow urological consultant Muhammad Naseem in not replacing a stent contributed to Mr Read’s death.
